On 18 September 2011 it was confirmed that the final, approved “Political declaration of the High-level Meeting of the General Assembly on the Prevention and Control of Non-communicable Diseases” – did indeed, include references to renal disease wherein it states, “We, Heads of State and Government and representatives of States and Governments, assembled at the United Nations from 19 to 20 September 2011, to address the prevention and control of non-communicable diseases worldwide, with a particular focus on developmental and other challenges and social and economic impacts, particularly for developing countries – [paragraph 19]
“Recognize that renal, oral and eye diseases pose a major health burden for many countries and that these diseases share common risk factors and can benefit from common responses to non-communicable diseases”
Dan Larson has been instrumental in coordinating many activities, leading to this success.
Dan was able to ‘mobilise’ a network of ISN members and other colleagues in the renal
community, including those activated through IFKF. Many were able to make contact with their own health ministers, and in turn a number of those health ministers wrote directly to WHO advocating for the inclusion of kidney disease in the final statement. These included health ministers from Mexico, Ethiopia, China, India, Japan, the United States – and several others.

Guillermo Garcia Garcia has provided us with two important documents. The first one is a PAHO publication on Cardiovascular Health in the Americas, where at last CKD screening in high risk populations is included as an important strategy for the prevention an control of CKD and CVD (Item 56). Another one is the inclusion of renal disease (item 19) in the UN Declaration on the Prevention and Control of NCDs. Both are the result of the efforts of ISN, IFKF and SLANH, among others, lobbying national governments to include CKD as a major CVD risk.

IFKF is a member of the European Kidney Health Alliance EKHA. The membership has been sponsored by the Dutch Kidney Foundation. CEAPIR, representing European patient organisations is also a member, and is represented by Mark Murphy, CEO of our Irish member. EKHA organizes every year the World Kidney Day event in the European Parliament, together with the WKD team.

The Members of Parliament (MEP) Group for Kidney Health met with the MEP Heart Group, MEPs against Cancer and the EU Diabetes Working Group for an unprecedented meeting on chronic diseases in Europe.

Experts from the Chronic Disease Alliance presented policy recommendations from the paper ‘A United Prevention Approach’ and the chairs of the four MEP Groups issued a joint statement calling for action on improved policies on the four main risk factors for chronic diseases: tobacco, poor diet, alcohol and lack of physical activity.

Prevention of cardiovascular diseases
Dr Maria Bianchi from Argentina has provided us with an excellent document on the prevention of cardiovascular diseases, with references to the detection and prevention of CKD. The document is written in Spanish and English. It is a Regional Plan for Action’ for the period 2011 – 2016. The plan is the result of the work of a panel of specialists during a meeting in Chaco, organized by the Argentine Northeastern Kidney Foundation, with the participation of Dr. Miguel Riella, President of IFKF. The document has been edited by the Organizacion Panamericana de la Salud (PAHO). This document is also very relevant for other regions in the world.

Registration fee subsidy: Participants who can not get any support from companies, ERA-EDTA etc. may apply for Hungarian Kidney Foundation support.
The course is designed to meet the requirements of 30 European/US CME credit points. For Hungarian participants the course is accredited by OFTEX for 50 credits. The Course included 43 hours of scientific lectures and a final examination which can be locally evaluated according to national standards.

Congratulations to The Tanker Foundation who celebrate 17 years of service - to 30 June 2010
Achievements include:
- 609 patients 70,460 dialysis sessions have been given at subsidized rates
- Rs. 58.65 Lakhs disbursed to 988 patients as one-time contributions ranging from Rs. 5,000/- to Rs. 50,000/- for dialysis, transplantation and medication costs
- conducted over 152 Awareness Programs about kidney disease in school, colleges & public forums.
Future plans
- From July 1 2010, they reduce cost of a dialysis at our subsidised dialysis units at Kilpauk, Ambattur and Madurai. Patients will now be charged Rs 375 per session.
- Donors can sponsor a dialysis at Rs 800 as this is the cost to the foundation.
- Our website (wwww.tankerfoundation.org) will be revamped later in the month.
- Set up a dialysis unit in Coimbatore in a space donated by Mr Krishna Kumar of Arya Vaidhya Pharmacy

The IFKF 'Sister' Program
A refreshingly innovative program for our IFKF Council Members to consider, aimed at enhancing cooperation among Kidney Foundations worldwide. Its mission is to facilitate the cooperation between developing Foundations and centers of excellence and the encouragement of exchange of personnel, resources and strategies.
Format - The essence of the Sister Program is to promote the development of a young and/or smaller Foundation (EF = Emerging Foundation) through the goodwill and resources of the Supporting Foundation (SF).
Benefits - The SF will have the advantage of recognition in IFKF media (IFKF news, e-update and website) and can also capitalize on the strengths of the EF based on local experience and locally prevailing clinical and environmental conditions. A EF will have preference for travel grants to our Annual Meeting and to register at a discount rate.
First time application - Any Kidney Foundation can form an IFKF Sister Center pairing, provided that:
* at least one unit is in the developing world and is considered the Emerging Foundation
* both units are IFKF Council Members
A certificate will be given by the IFKF at our Annual Meeting to both organizations to knowledge the partnership.
The application system is an online form that allows the main contact person at both centers to access and fill-out the application information. You are required to provide the two valid email addresses of the contacts at each center.

The Joel D Kopple Award
This newly created award recognises IFKF Council Member organisations, individual Members, or Associate Members, for outstanding performance in the area of kidney disease – detection, prevention, treatment, care. This Award will be presented annually during future IFKF Annual Meetings.

The Jury will be formed by a panel of the individuals appointed by the Board and will consider the following criteria:

Outstanding performance refers to any initiative or action resulting in a considerable and visible short term or long term internal or external impact.

Internal impact can be growth in finances, membership or other organizational attributes, improved management, professionalizing etc of the organisation.
-
External impact can be any initiative or action resulting in the improvement of detection and prevention of chronic kidney disease, treatment of kidney disease or care of the kidney patient.
-
Measurement of outstanding performance will be considered in the context of the local environment and resources (income, financial resources, manpower, skill sets) available to the nominated organisation or individual at the time of the contributions.

Ensuring kidney relief for the victims of the Haiti earthquake

An ISN Renal Disaster Relief Task Force (RDRTF) assessment team entered Haiti via Santo Domingo, flying from Paris on 15 January. The team works in close collaboration with Médecins Sans Frontières (MSF). A second team with further equipment reached Port-au-Prince on 17 January. A third team was also sent later to Haiti.
All team members carried light material (e.g. Kayexalate, dialysis catheters) in their hand luggage. A cargo with heavy material (4 dialysis machines, an inflatable hospital) left Europe on 16 February but it was not possible for the carrying plane to land in Haiti. The material was rerouted to Santo Domingo in Dominican Republic, to be shipped to Port-au-Prince on smaller planes, eventually arriving January 17.
All the time we were confronted with serious communication problems, whereby only sporadic contacts can be held by satellite phone (early morning, local time) and email (late evening, local time). No contact is up to now possible by mobile phone. The situation with which our team is faced is dramatic. Most wounded people are lying in the streets without care, although the situation is improving slightly. All local hospitals are overcrowded and there is a shortage of all kinds of medical material including drugs, as well as water and electricity. Identification, triage and transport of victims with potential crush are hard to organize.
Our team found a hospital with three intact dialysis machines [General (University) Hospital] which were no longer used (all chronic patients had left when the earthquake occurred and did not return). To the best of our knowledge, this is the only potentially operative dialysis unit in the city. The building had first to be declared safe. The planning was to make the unit operational then add our own four machines. MSF would generate its own water and electricity. The whole process was unfortunately delayed due to chaos. The three Haitian machines could not be made operational as some materials were locked in closets and the local nephrologists could not be reached to open them. Once our cargo arrived in Port-au-Prince we had problems with transport and unloading.
It goes without saying that these are the most extreme disaster conditions we have ever been faced with during the existence of the RDRTF. Fortunately, we can rely on the excellent logistic support of MSF. We also are grateful for all the help and support we received from colleagues from all over the world. But structural chaos together with a disaster of unseen dimensions makes progress extremely difficult. Nevertheless, all these tiny acts of support together will help save a substantial number of lives.
The ISN President, Bernardo Rodriguez-Iturbe, is personally involved in coordinating efforts across the various scientific societies and organizations (SLANH, the Dominican Society of Nephrology, some of the dialysis manufacturers and of course the ISN members from Haiti and Dominican Republic).
