Chronic Kidney Disease in the United States: A Public Policy Imperative

Results: Despite new evidence-based therapies to slow, stop, or reverse the progression of CKD to ESRD as well as premature cardiovascular disease, major systemic barriers continue to limit the implementation of this body of evidence at the level of the nephrology practice. Key factors include under- or uninsurance, unstructured medical care systems, and lack of enabling public policies.

Conclusions: The crisis of nephrology is embedded within the unresolved duress of the ability to provide quality early intervention juxtaposed upon inadequate reimbursement for clinical care and procedures, unfunded mandates for information technology systems, and organizational inconsistencies between nephrology and other specialties. We believe now is the time for the renal community and related stakeholders to unite in an effort to address the clinical, financial, and public policy issues that will enable the delivery of appropriate CKD care to this vulnerable patient population.

IFKF is proud to announce 4th National Convention of the Kidney Foundation of Bangladesh on 25 November 2008 and wishes its member a successful event.

This Convention will highlight "Recent advances in the treatment and Prevention of Chronic Kidney Disease" and features two lectures on "Role of stem cells in the prevention of CKD". The Kidney Foundation of Bangladesh has expanded our dialysis facility from 14 dialysis machines to 20 and will be further expanded to 30 in two years time.  They are also doing transplants 1-2 times a week and treatment is provided on Not for Profit basis.

Conclusion:  Undoubtedly, dialysis is very expensive, but it is also a life-saving treatment. There is clearly room for efficiency improvement by rational strategies. Cost analysis reveals both bright and dark spots, calling for realistic implementing policies to improve treatments with cost-effective strategies.

Satellite/home HD may be an appropriate choice to decrease the overhead and personnel costs in places where they are main cost drivers (Western world). A major attempt should be made to increase PD coverage, as a comparable and possibly cheaper modality of RRT in many countries with limited budgets,1 particularly if we cannot keep all chairs full. Equally, we should mainly work on local production of bicarbonate powder and also encourage reuse in Iran and dialyzers in other countries with high numbers of ESRD patients.

On the one hand, 5 countries—the United States, Germany, Italy, Japan, and Brazil—with 12% of the world’s population—already treat more than half of the world’s people with ESRD. More than 100 developing countries, together with a population in excess of 600 million, do not have any provision for renal replacement therapy. On the other hand, patients with ESRD comprise only a small percentage of people with chronic kidney disease and the total prevalence of chronic kidney disease may be 50 times that of treated ESRD.32 Considering CKD prevention, by initially focusing on strategies and treatments that slow disease progression, we can postpone RRT to save national and family resources

Declaration of Istanbul on Organ Trafficking and Transplant Tourism

Publication relates to Dr Delmonico's presentation - 9th IFKF Annual International Meeting, Germany. September issue..

Journal Nephrology Dialysis Transplantation (NDT) has published this declaration with commentary by Dr Delmonico. The Turkish physicians who are contributing to our Istanbul Meeting mAY 2009 were both participants, with Dr Mehmet Sever on the steering committee. All countries should take steps to govern organ donation and transplantation, thereby ensuring patient safety and prohibiting unethical practices. Declaration of Istanbul on Organ Trafficking and Transplant Tourism is a consensus of more than 150 representatives of scientific and medical bodies from around the world, government officials, social scientists, and ethicists, who met in Istanbul, Turkey.
