Status of Mircera |
By Jim Scherrer on
10/7/2008 |
We are US citizens living in Puerto Vallarta, Mexico for the past 11 years. My wife was on dialysis for 5 months but has been off for the past two months and her creatinine is holding steady at 3.5.
The nephrologists here in Mexico have prescribed Exetin-A whereas we used Procrit, Epogen, and Aranesp in the US. While she is off dialysis, I administer the EPO at 10,000 i.u.´s/week and maintain her hemoglobin at 12 with no problem.
The EPO alpha burns like hell and my wife would really like to try Roche´s new miracle drug, Mircera, for a while. br> Can anybody help us in finding a supplier of Mircera?

Blogosphere ideas from Uruguay |
By Claudio Orazietti on

I´m the Vice-President of the Sociedad Uruguaya de Nefrología www.nefrouruguay.com. In the lasts years with the FREU (Fundación Renal del Uruguay) and ATUR (Asociación de Transplantados del Uruguay) worked for World Kidney Day. Both the mentioned organizations are Members of the IFKF. You can see some activities described in Pasts Events - Uruguay at the WKD web page.
In November we held a Conmemorative Celebration of the 50 years of the first hemodialysis in our country. The Activities were at the Hospital de Clínicas at the Centro de Nefrología de la Facultad de Medicina de la Universidad de la República. There was a lot of emotion with the participation of the Prof Emmeritus Dr Dante Petrucelli (who participated 50 years ago in our first hemodialysis). Dr Miguel Riella participated of the cellebration with speaches about his Kidney Foundation and clinical topics. At any moment we remind you of our WKD efforts! We think this conmemorative events allows us to have a new opportunity to send to our people the messages of the relevance of the care of the kidney function. WE WANT TO STIMULATE OTHER ORGANIZATIONS IN OTHER COUNTRIES TO TAKE IN ACCOUNT THIS EXPERIENCE. Now we are working for the 2009 WKD. It brings some difficulties to organize effectives activities, but we continue. Soon we will send information. I logged the WKD Forum and send a message. With kind regards - Dr Claudio Orazietti |

Hi, I am from Malaysia. My father is undergoing dialysis everyday. I would like to know is it illegal for Malayisia government servant to have transplant of both of his kidney. Both of my father kidney have failed to function. He is a senior civil servant and according to his colleague, if he undergo a kidney transplant, then he would lost his retirement fund as it is not ethical for civil servant to have organ transplant.
I really hopes someone from IFKF can help me to solve my problem. |

Dear Aneeshshankar - unfortunately the IFKF can not offer financial support to the millions of people who are in the same situation as yourself. Seeing your family in pain knowing that medicine can help them, but being unable to supply that is a terrible burden on anyone. The IFKF are trying to work with Government and departments that manage public health to increase prevention and early detection efforts globally. As an organisation was do not receive massive funding. Our Members pay fees which operate the IFKF. I hope you and your family find peace in their life. Regards - IFKF Web Manager |

The Epidemic Of CKD IN Southern Sudan |
By Valentine Imonje on
12/1/2011 |
A few days ago, I was in the state of Western Bahr El Ghazal, which is just two hours away, by air, from Juba in the Southern Sudan.
I was following up on a group of community health workers I had trained. I wanted to find out if the strategies we had laid had worked.
Previously, I had given them some tough love and made it clear that if they wanted to be part of our organization, they each must identify at least 5 clients who are living CKD.This is a country that is just recovering from deadly war and health systems are either not working or not there at all.
They vowed that this was impossible, citing high levels of literacy and lack of proper medical facilities and low awareness of CKD, but I remained firm because I believe in service delivery, regardless of the obstacles.
Some of the excuses I hate are "it's impossible" and "I can't".
When they realized that I was not bluffing, they did a perfect job. Each identified at least some clients. Some even had more.
They even went a step further and formed links with the state hospital that provides minimal RRT with the support of a WHO staffer.
On this visit, I decided to go the hospital and see where clients were being referred to, what kind of assistance they were getting, and what more we needed to do to improve the partnership.
On reaching the hospital, which is in Wau, I saw dilapidated red-earthen buildings that escaped the war and which looked like they would crumble any minute.
Outside the buildings were groups of people huddled around small fires.
"Who are those people who have lit fires in the compound?" I asked the field officer who accompanied me. "How come they are allowed to light fires in the hospital's compound?"
The officer was surprised at my questions. "Those are patients preparing their meals," she replied, adding that the hospital does not have the luxury of a kitchen and that patients have to prepare their own food. I was shocked.
I was even more shocked when I learnt that relatives have to travel to the hospital very early, or stay overnight so that they can buy their patients water from a tanker that passes by daily. It is either that or you die of thirst.
The hospital's only toilets are pit latrines with an over-powering stench.
There were also no bathrooms to speak of. It is almost a normal thing in Southern Sudan that if you are out in the open countryside, you can go on and take your bath under the cover of darkness.
And this is not only for the sick; it is also the done thing for the general populace. No one will bother to look at you because bathing out in the open is common around here.
With us was a woman who was in critical condition and need treatment urgently. We tried to see if she could be admitted, if not in the "hospital", then in the compound so that she could wait in the queue for the nephrologist(The only one in the Whole State).
I personally intervened. I am known there as wa mshikila.
I understand mshikila is any kind of trouble and I am labeled this way because of my focus on CKD. The Nephrologist had heard about me in Juba.
This woman's sole caregiver was her seven-year-old daughter, who was already overwhelmed by the responsibility.
If the doctor listened to our case, we would, hopefully, pre-empt this woman's impending demise and remove the burden of care from her child.
After a lengthy consultation, she was admitted. Following the procedures, we had to go and purchase a reef mat to serve as her bed, a few utensils for her makeshift kitchen, and supplies, even though she had no energy to cook.
We agreed that the community workers would go to the hospital on alternate shifts to cook for her. I must say that it was quite a relief that she was finally admitted.
However, our relief was short-lived because the following day, as I waited for my plane back to Juba, which was delayed because of lack of fuel, I was informed that she had passed away. Mind you I have witnessed these kind of deaths that as looked at as normal but they are indeed sickening and unnecessary.
My heart broke, especially for her young daughter, who was used to having her mother around.
My heart also went out to the community workers, faced with the unenviable task of delivering the terrible news that her mother was no more. It is not an easy job, this one.
As we prepare for World kidney Day let us all think about the people who need treatment and yet cant get for one reason or another. Let us pledge to work and do whatever we can to help make treatment available to all CKD patients.
